How The Service Works
At Hope Care Service Agency, we curate unique strategies and a delivery plan that support both the care recipient and their family. We are here to fully support the individual, regardless of health care needs.

Consultation and Care Assessment
A consultation and care assessment will be carried out by the Registered Manager, to put together a tailored and bespoke care package specific to the individuals needs and requests. We guide a conversation to assess the type of carer that would best suit the care recipient, as well as the support required. A conclusion of the consultation and care assessment will then be written as a ‘Care Plan’, which is essential for both the service user and visiting carer.

Care Set-Up
The Registered Manager will bring the chosen carer to meet the service user, and to formally introduce them and to talk the carer through the care plan and support required. The Registered Manager will ensure that the carer fully understands the care delivery requirements, and to make certain the highest standards will be met.

Ongoing Support
A dedicated team is on hand to anticipate the service user’s care needs and their emotional support throughout the process. Where necessary, we can redesign the care plan, put a different carer in place, rearrange times to suit the service user’s preference, whilst always maintaining the highest standards of care delivery.

Supporting in personal care needs, and providing these in a discrete and respectful manner always with dignity.

Communication
Clear, concise and emotionally sensitive communication, both written and verbal.

Support and encouragement of the service user’s social life, whether it be visiting friends or family, or being taken out by the carer.

Encouraging and promoting independence, and ensuring the service users safety in moving around the home and outside of the home.
Housekeeping
Washing of clothes and bed linen and carers ensure they clean and tidy up after themselves, to provide a healthy and comfortable living environment.

A specially trained carer will provide tailored support.
